heal.abstract	Η τεκμηρίωση της παρεχόμενης νοσηλευτικής φροντίδας αποτελεί ζωτικής σημασίας κομμάτι της νοσηλευτικής διεργασίας. Το 38% του ημερήσιου νοσηλευτικού χρόνου αφιερώνεται στην καταγραφή του εκτελεσθέντος νοσηλευτικού έργου. Η νοσηλευτική τεκμηρίωση γίνεται προκειμένου να εκπληρωθούν διοικητικοί και κλινικοί σκοποί. Στη νοσηλευτική βιβλιογραφία αναφέρονται επτά τρόποι νοσηλευτικής τεκμηρίωσης: αφηγηματικού τύπου σημειώσεις, τα αρχεία που είναι προσανατολισμένα στο πρόβλημα, τα αρχεία που είναι προσανατολισμένα στην πηγή, η κατ' εξαίρεση καταγραφή, τα Kardex, η κατά περίπτωση καταγραφή και τα ηλεκτρονικά αρχεία. Οι κανόνες για μια πετυχημένη νοσηλευτική τεκμηρίωση είναι πολλοί, ωστόσο στο επίκεντρο παραμένει η ποιοτική νοσηλευτική τεκμηρίωση προκειμένου να εξασφαλιστεί η επαρκής και εξατομικευμένη φροντίδα των ασθενών. Η επίλυση των προβλημάτων των ασθενών απαιτεί τη συνεργασία μεταξύ των μελών της υγειονομικής ομάδας. Ο γραπτός λόγος παραμένει στο επίκεντρο της επικοινωνίας ως ο σαφέστερος, πληρέστερος και νομικά κατοχυρωμένος τρόπος επικοινωνίας στον χώρο της υγείας.	el
heal.abstract	The documentation of the nursing care is an essential part of the nursing process. The 38% of the daily nursing time is spent for docu¬menting the nursing care that it has been provided. Nursing documentation has two professional purpo¬ses: administrative and clinical. In the nursing bibliography there are mentioned seven types of patient records: the narrative form, the problem oriented medical records, the focus charting format, charting by exception, kardex, clinical path-ways and the computer based documentation. The rules for a successful nursing documentation are ma¬ny, but the focus remains to be the quality of charting in order to reassure the adequate and personalized care of the patients. In order to solve the health pro¬blems that the patients face there must be a successful communication within the heath care team. Writing remains in the center of a successful com¬munication as a clear, complete and legally secure way of communication in the health care environ-ment.	en
