heal.abstract	Η υπογλυκαιμία που δημιουργείται σαν παρανεοπλασματική εκδήλωση οφείλεται στην παραγωγή των ινσου- λινομόρφων αυξητικών παραγόντων (IGF-I και IGF-II) πού παράγονται από όγκους των μη νησιδιακών κυττάρων και παρεμβαίνουν στο μεταβολισμό της γλυκόζης προκαλώντας υπογλυκαιμία. Η άμεση αντιμετώπιση συνίσταται στη χορήγηση σακχαρούχου ορού, γλυκαγόνης και υδατανθρακούχου γεύματος. Επιπλέον συνιστάται αφαίρεση του όγκου. Νεώτερα δεδομένα στρέφονται στην ανακάλυψη αντι- σωμάτων που αναστέλλουν την σηματοδότηση του υποδοχέα IGF-IR και αποτελούν μια πολλά υποσχόμενη αντινεοπλασματική στρατηγική. Ως εκ τούτου σε περιπτώσεις ασθενών που εμφανίζουν υπογλυκαιμία άνευ εμφανούς αιτίας πάντα πρέπει να εγείρεται η υπόνοια παρανεοπλασματικής εκδήλωσης και να γίνεται ενδελεχής έλεγχος. Xαμηλή στάθμη ιν- σουλίνης στο ορό σε συνδυασμό με αυξημένα επίπεδα της μεγάλης IGF-II και αύξηση της σχέσης IGF-II/ IGF-I επιβεβαιώνουν την διάγνωση.	el
